Evidences in the UniProtKB flat file format

The evidence for annotations in UniProtKB entries has been available for several years in the XML and RDF representation of the data and we have now added this information also to the text format (aka flat file format).

Changes to the controlled vocabulary of human diseases and keywords

New diseases and keywords added. Diseases have been modified and also deleted. See details at http://www.uniprot.org/help/2014/10/01/release.
